Please carefully read through this contract to the bottom. At the end, you will find a link to the Adoption Application. You will be asked to sign a hard copy of this contract upon adopting the pet you choose.

I WISH TO ADOPT A PET FROM THE CITY OF ANGELS DOG RESCUE AND I AGREE TO THE FOLLOWING CONDITIONS:

I will be responsible and provide proper food, shelter, water and kind treatment at all times.

I will not abandon or give away the pet I have adopted to any other party. I will not relinquish my pet to any other organization or shelter or dispose of him/her in any way unless I first notify the City of Angels Dog Rescue.

I agree to take the pet I have adopted to a practicing veterinarian within 7 days from this date for a physical examination and I agree to provide regular veterinary care for the life of my pet. 

I agree to obey all city and county leash laws as they regard my pet.

I understand the dog or cat I have adopted is to be a HOUSEHOLD PET only, where he/she will be kept as part of the family unit.

I agree that the dog I have adopted will not be chained to a dog house or post or used as a guard dog.

I understand that the City of Angels Dog Rescue does not adopt animals to homes without a proper home check.

I understand that animals are different from human beings and that their actions and responses can be unpredictable.

I understand that the City of Angels Dog Rescue strives to place healthy animals for adoption but they cannot guarantee the health of any pets.

I understand that the City of Angels Dog Rescue makes no claims or representations as to behavior or temperament of any animals placed for adoption.

I am in agreement that a follow-up visit may be made to my home by the City of Angels Dog Rescue and that any violation of this contract is good and sufficient reason for reclaiming of said pet by the City of Angels Dog Rescue.
